Scaffold stability as demolition progresses

I came across this news today, and I was shocked and angry. Why? Because there was another collapse at this very site only a few weeks ago. How could this happen again if the contractor and the relevant authorities had properly investigated the first incident and dealt with the risks?
The building is being demolished. Looking at the footage, my immediate question is: how was the scaffold restrained as the demolition progressed? If the parts of the building supporting its ties were being removed, what was put in place to keep the scaffold stable?
Where the building can no longer provide reliable support, a properly engineered stand-alone scaffold should be considered. You cannot simply remove the support and hope the scaffold will withstand a strong wind.
The exact cause of this collapse still needs to be established. But a second collapse at the same site should never have been allowed to happen.
